RESUMEN

PURPOSE: We studied the views of nursing students on family-centered care (FCC) and their knowledge, opinions, self-rated competence, current practice, and perceived implementation barriers with regard to trauma-informed pediatric nursing care. METHODS: This survey was a descriptive correlational study. The sample consisted of 261 nursing students, 3rd and 4th years, who had completed the Child Health and Diseases Nursing Course. The data were obtained using the "Student Information Form," "Family-Centered Care Attitude Scale," and "trauma-informed care (TIC) Provider Survey." RESULTS: Nursing students were knowledgeable and held favorable opinions about TIC. The survey showed that students with higher levels and those with a hospitalization experience during childhood had higher scores regarding TIC. A positive relationship was found between the students' TIC to mean score and FCC attitude mean score. CONCLUSIONS: Nursing students are not competent to practice TIC, especially with pediatric patients. Therefore, they need to develop relevant skills for helping pediatric patients. PRACTICE IMPLICATIONS: Efforts to improve nursing students' trauma-informed pediatric care should highlight specific skills related to helping pediatric patients manage emotional responses to difficult medical experiences. By integrating TIC into the baccalaureate curricula, nursing educators can provide the students with appropriate skills and facilities so that they can provide holistic and highly effective care to highly vulnerable patients.

Na Revisão da Literatura, incluíram-se artigos, publicados nos últimos cinco anos e disponíveis gratuitamente, através dos seguintes descritores: "meaning in life" or "life purpose", "depression", "nurs*" or "family nurs*", "hospital" utilizando o operador booleano AND, OR e NOT.

Family nursing emerges as a distinct phenomenon for both generalist and advanced nursing practice. The theoretical foundations are based on systemic and interactional thinking. A systemic approach and collaborative support relationship are used in the care of the family to ensure competent care and excellence in nursing interventions, which take place reciprocally in a context of intersubjectivity and complexity. Family represents a powerful source of support for an ill person, it is inherent to the care process and it is intrinsically involved in the health/disease process. That is why it is of paramount importance the concern and commitment that the family is integrated into health care, ensuring the promotion of family health. The evidence of the gap between the theoretical and the practical in terms of nursing led to a study on knowing the attitudes of nurses and students regarding the importance of the family in the care process, the systemic approach to family care, in professional performance. This is a quantitative, descriptive, analytical, and correlational study, carried out with a sample of 7 nurses who practice in a Family Health Unit and with a group of 22 students attending the degree in nursing. As a data collection instrument, the sociodemographic questionnaire and the FINC-NA scale were used, validated for the Portuguese population by Oliveira et al., 2009. In the data obtained, in relation to the sociodemographic and professional variables that characterize the sample, it was only possible to verify an association between the attitudes of nurses and students, regarding the importance of the family in the care process, in the variables gender and professional title. It was understood that a family is a complex unit, with needs and impact on the health state. With evidence, we can conclude that in the life cycle transitions, families lack the holistic approach of family nurses. Family nursing has these skills to take care of families as a system, assuming a central position, being essential training to provide nurses with knowledge, tools, and efficiency in this optimization of care.

Na Revisão Sistemática da Literatura, incluíram-se artigos, publicados nos últimos seis anos e disponíveis gratuitamente nas bases de dados Academic Search Complete, Complementary Index e Medline, através dos seguintes descritores: Informal "caregiver burden", "COVID-19 or coronavirus or 2019-ncov or sars-cov-2 or cov-19", "life purpose" or "meaning in life" or "purpose in life", utilizando o operador booleano AND.

There is currently a high concern regarding the impact of the COVID-19 pandemic on the general wellbeing of the population. This pandemic constitutes a global crisis, not only in terms of public health and economic stability, but also of family well-being. By evoking questions about the meaning of life, the pandemic constitutes an opportunity for the discovery of human and universal values. In an increasingly aging society, the COVID-19 pandemic has further accentuated the difficulties experienced in the daily lives of caregivers at various levels, namely, professional, personal, financial and social. Viewed not only as the basic unit of society, but also as the focus of nursing care, the family has, as a priority function, the support and protection of its members. Due to their proximity, the family health nurse knows the singularities of ach family, their potential, their skills and weaknesses. With his technical, scientific and humanistic knowledge, he defines strategies of a systemic nature that enhance the action of the family, given the specific needs of each of its constituents. However, it is urgent to understand what are the challenges, responses and adaptations that emerged from this new reality, since the pandemic scenario has conditioned the interaction between nurses and families. Based on this purpose, a systematic literature review was carried out, underlying the following research question: What are the consequences of the COVID 19 Pandemic on the burden and meaning of life of the informal caregiver? The objectives defined for the research are: 1) To assess the burden of the informal caregiver during the COVID-19 pandemic period and 2) To identify the relationship between the meaning of life and the burden of the informal caregiver. From the PI[C]O question, keywords were defined and a search for scientific articles was carried out in the B-On platform database in November 2021. In the Systematic Literature Review, articles published in the last six years and freely available in the Academic Search Complete, Complementary Index and Medline databases were included, using the following descriptors: Informal "caregiver burden", "COVID-19 or coronavirus or 2019-ncov or sars-cov2 or cov-19", "life purpose" or "meaning in life" or "purpose in life", using the Boolean operator AND. From the application of the inclusion criteria, a total of 58 articles were identified, of which, after analyzing the titles and abstracts, five articles were selected as eligible and included. Regarding the results, it was found that caregiver burden varies according to the illness of the person cared for, and evidence shows that caregivers of people with dementia and cancer reported a greater burden of care, especially for women. Anxiety, chronic stress and depression (anxiety and existential suffering) were also generators of the caregiver's feeling burden during the COVID-19 pandemic. It was found that promoting resilience significantly reduces care-associated distress. It is concluded that the presence of spirituality, hope and a life propouse, along with resilience, significantly reduces the anguish associated with care. It is essential that caregivers are properly trained and qualified so that they reach greater levels of resilience for the ability to accept the situation on a daily basis, meeting the need for the person not to lose their essence and restore hope. It is crucial that nurses develop skills to identify and intervene in the health-disease transition processes, guiding caregivers to reach the level of mastery, promoting resilience and hope.

he path taken towards the acquisition of competences in Family Health Nursing, recognized as a deepening and mobilization of professional experience and theoretical knowledge, in different contexts of practice, is reflected in this document. The purpose of life is a philosophical and an existential question about the purpose and meaning of human existence. If, on one hand, spirituality has been acquiring increasing importance with regards to the individual's quality of life, on the other hand, it can be reflected in their health. It influences the way they act towards life events, leading them to feel the need to care for and preserve their health or, in the case of illness, to be resilient and face this status with an optimistic attitude. In this way, a was developed Scoping Review (ScR), directed to the theme of the meaning of life and forces, using the PI[C]O method and taking as the question: "Is the Family Health specialist nurse sensitive to the purpose of life as added care to the patient/caregiver?". Data collection includes studies related to the period of time between January 2015 and December 2020, with six studies selected from the search in electronic databases (PUBMED, EBSCO, CINAHL, MEDLINE and OPENGREY). This subject carries great responsibility. If, on one hand, nurses create their own identity, build their reality and make it viable as a source of life, on the other hand, for some, nurse's work is associated with an existential void resulting from the impossibility of attending to their own motivations. Faced with the constant atmosphere of suffering, pain and incapacity when working with caregivers and families, who places all their trust in nurses, leads them to try to understand the strength and meaning that each one seeks for himself. In the interpretations of the studies, it is perceived that the purpose of life/strengths of caregivers and families is present and influences the work of nurses. Related to the spiritual dimension, this is conveyed by the process of Self Transcendence, by the discovery of intrinsic forces and has the potential for a personal and professional transformation of each nurse. It highlights your "being" and directly contributes to a better development in your work. The result translates into potential benefits for the caregiver, family, and community. Thus, the practice of caring in this Master's subject should not be limited to scientific and technical knowledge, but rather extrapolate to another dimension that is equally or more importance, which corresponds to the relational aspect. All the learning, experienced in the different placements' contexts allowed this understanding and converged to a personal and professional enrichment.

RESUMEN

BACKGROUND: The need for palliative and hospice care for persons with life-limiting conditions who are incarcerated is increasingly common in correctional facilities worldwide. OBJECTIVE: Through a family nursing lens, we critically analyze the unique challenges experienced by those requiring palliative care while incarcerated. Key concerns and implications for nursing are illustrated through the discussion of a representative case scenario. METHODS: The case scenario was developed based on the findings of a scoping review of academic and gray literature (such as policy documents and editorials) about palliative, hospice, and end-of-life care in corrections. RESULTS: The case scenario highlights multilevel barriers that were identified, including the individual, relational, institutional, and sociocultural contexts of palliative care in correctional facilities. Evidence is presented of how integration of a family nursing perspective can enhance specialized palliative and hospice care services in correctional settings. CONCLUSIONS: By applying a family nursing approach, nurses practicing with correctional populations can ensure persons with life-limiting illnesses are not denied their right to appropriate end-of-life care by playing a key role in addressing barriers to palliative and hospice care access in corrections. Specific attention to relational issues and holistic care can contribute to enhanced palliative and hospice care, greater dignity in dying, and improved family and peer outcomes, which have benefits for individuals, families, communities, and society. IMPLICATIONS FOR PRACTICE: Through the case scenario, we illustrate real issues emerging in correctional contexts and offer evidence of how family nursing relational principles can be applied to promote adequate palliative and hospice care.

RESUMEN

BACKGROUND: The Family Model is increasingly used as a framework to promote a whole family approaches in mental health services. The purpose of this qualitative study was to (i) explore whether nurses practice in accordance with the Model when providing services for parents who have mental illness, (ii) determine whether components of the model are employed more in some workplaces (i.e. acute in-patient units versus community settings) and (iii) ascertain reasons as to why this may be the case. METHOD: A purposive sample of 14 nurses from eight mental health services in Ireland, completed semi-structured interviews. RESULTS: Findings indicated that nurses' practice incorporated most key components of The Family Model, including supporting service users' dependent children. However, some practices were not explicit in the Model, such as supporting other adult family members (i.e. grandparents). While nurses' practice in community settings was more aligned with The Family Model than in acute in-patient units, there was a notable absence of reference to domain six (cultural considerations) in both settings. A holistic and family-centered philosophy, coupled with collegial and managerial support and a focus on prevention were the main features that enabled family focused practice in community settings. CONCLUSION: As only a subgroup of nurses practice in accordance with The Family Model, efforts are required by nurses, their organisations and the broader system to promote family focused practice. The Family Model may be a useful framework, with further refinement, for guiding nurses' practice.

RESUMEN

BACKGROUND: Although primary care settings provide a large-scale and high-reach opportunity for weight management and obesity prevention, the proportion of adults in the United Kingdom (UK) who report receiving weight management advice is limited. This study examines the self-reported frequency of assessing weight and providing weight management advice by General Practitioners (GPs) and Practice Nurses (PNs) working in primary care in the UK, and differences by practitioner characteristics. METHODS: Cross-sectional survey with GPs and PNs in the UK (n = 2020), conducted January-March 2017. A mock consultation exercise assessed what factors led to calculating a patient's Body Mass Index (BMI) and whether weight management advice was given after determining the patient had an obese BMI. For all patients, practitioners were asked how often they calculated BMI, how often they gave weight management advice to patients with an obese BMI, and how often they utilised different advice or referral options (each: Always/Often vs. Less often/Never). Binary logistic regressions examined whether frequency of assessing weight and providing advice was associated with practitioner characteristics. RESULTS: In the mock consultation, physical cues (40%) were most likely to prompt calculation of BMI, and half of practitioners (56%) provided weight management advice after determining the patient had an obese BMI, with GPs less likely to do so than PNs (Odds Ratio [OR] = 0.59, 95% CI: 0.47-0.75). Half of practitioners (58%) said they calculated the BMI of all patients Always/Often, with GPs less likely to do so than PNs (OR = 0.27, 95% CI: 0.21-0.34). Three quarters (78%) said they provided weight management advice to patients with an obese BMI Always/Often, with GPs less likely to do so than PNs (OR = 0.63, 95% CI: 0.47-0.85). Weight management advice was provided more frequently than referrals, particularly suggesting increased physical activity (93%) and diet modification (89%). CONCLUSIONS: Consistent with previous research, the findings suggest that opportunities to provide weight management advice in primary care, including to patients with an obese BMI, are potentially missed. Future research should test alternative mechanisms to increase weight assessment and advice provision, examine the effectiveness of advice frequently given, and seek solutions to reported barriers for providing weight management advice.

RESUMEN

Parent-infant separation is a major source of stress for parents of hospitalized preterm infants and has negative consequences for infant health and development. Family Integrated Care (FICare) uses a strengths-based approach, based on family-centered care principles to promote parental empowerment, learning, shared decision making, and positive parent-infant caregiving experiences. Outcomes of FICare include increased self-efficacy upon discharge and improved parent-infant relationships and infant developmental outcomes. In this article, the authors describe the FICare model and emerging evidence regarding outcomes of FICare for infants and families and discuss challenges and opportunities in implementing and maintaining high-quality FICare.

RESUMEN

It is the mission of the Community and Family Nurse through an integral and holistic approach to accompany people from cradle to death in developing their health potential, and promote different family, work and social environments to facilitate this development. Throughout history, various international, European and national organizations have regulated the figure of the Community and Family Nurse, and now their functions, powers and professional performance are fully regulated. The Community and Family Nurse can respond to the needs of a changing population and take on new responsibilities in management and research. Their extensive basic and advanced skills gathered under a rigorous training programme, benefit the health system, the nursing profession, citizenry and its communities. Many challenges remain for the Health Departments of each Autonomous Region to make it possible for this specialty to develop its full potential for improving care.

RESUMEN

OBJECTIVE: to appreciate the strategies promoted by nurses in the context of child health surveillance relevant to early childhood development. METHOD: this is a qualitative study with an inductive thematic analysis of the data, based on the conceptual principles of child health surveillance, and developed through semi-structured interviews with Brazilian nurses working with families in primary health care. RESULTS: the nurses' strategies in favor of child health surveillance focus on actions that anticipate harm with continuous follow-up and monitoring of health indicators. The process of child growth and development is the basis for responses and benefits to health, connection with the daily lives of families, active search, articulations between professionals and services, access to comprehensive care, and intrinsic actions between promotion, prevention and health follow-up. CONCLUSION: child health surveillance actions developed by nurses with families involve knowledge sharing, favor the resolution of problems, increase child health indicators, and strengthen the relationship between health and children's rights, which support the promotion of development in early childhood.

RESUMEN

OBJECTIVE: To analyze the single therapeutic project and the nursing process regarding its specificities and intersection points from the interdisciplinary care perspective. METHODIntegrative review of the literature from articles available in the Lilacs, SciELO, MEDLINE and PubMed databases, published in Portuguese, English and Spanish from 2005 to 2015. RESULTS: It was identified 23 articles. From these, 17 on the nursing process, six on the single therapeutic project and one about multiprofessional residency. From the analysis, their specificities and intersection points that describe the alignment and similarities between them were identified in the primary and mental health cares. CONCLUSIONS: The single therapeutic project and the nursing process are aligned in health practices in primary and mental health cares. The multiprofessional residency allows this alignment among them, and the nurse contributes to the interdisciplinary care with the nursing process.

RESUMEN

Midwives and Nurses in Early Childhood Intervention: The Benefit of Additional Qualification Abstract. BACKGROUND: Early childhood interventions are locally and regionally organized support services for families from pregnancy until the end of the third year of life. The interventions promote diverse measures to enhance parental skills in order to improve developmental and living circumstances. Midwives and nurses with additional qualification support burdened families in early childhood intervention. METHOD: Within a retrospective survey (standardized interviews, CAPI) mothers' (N = 298) perspective of the benefit of the home visiting support is assessed. Data from two groups were compared: (1) mothers in the care of a midwife or nurse with additional qualification (GruppeGFK + Quali) and (2) mothers cared for by a midwife or nurse without additional qualification (GruppeGFK). RESULTS: (1) Families with weighted levels of psychosocial burdens reported an enhanced need for help. (2) Midwives and nurses with additional qualification support more frequently families with high levels of psychosocial burdens. (3) Mothers with care of midwives and nurses with additional qualification reported this support as more useful in relation to every day demands than mothers with regular care after birth (questionnaire for evaluation of the received support: GruppeGFK + Quali: mean = 2.57; GruppeGFK : mean = 1.97; t (121) = 2.799, p = .003). CONCLUSION: The study complements results of national and international studies showing that families with high levels of psychosocial burdens accept home visiting support. Furthermore, this support seems to be useful. An increase of the offer and the additional qualification is recommended for improving the developmental and living conditions of families with psychosocial burdens.

RESUMEN

BACKGROUND: Most Neonatal Intensive Care Units (NICUs) in China have restricted visiting policies for parents. This also implicates that parents are not involved in the care of their infant. Family Integrated Care (FIC), empowering parents in direct care delivery and decisions, is becoming the standard in NICUs in many countries and can improve quality-of-life and health outcomes of preterm infants. The aim of this study was to evaluate the impact of a FIC intervention on the clinical outcomes of preterm infants with Bronchopulmonary Dysplasia (BPD). METHODS: A pre-post intervention study was conducted at NICUs in two Chinese children's hospitals. Infants with BPD were included: pre-intervention group (n = 134) from December 2015 to September 2016, post-intervention (FIC) group (n = 115) and their parents from October 2016 to June 2017. NICU nurses were trained between July and September 2016 to deliver the FIC intervention, including parent education and support. Parents had to be present and care for their infant minimal three hours a day. The infants' outcome measures were length-of-stay, breastfeeding, weight gain, respiratory and oxygen support, and parent hospital expenses. RESULTS: Compared with control group (n = 134), the FIC group (n = 115) had significantly increased breastfeeding rates (83% versus 71%, p = 0.030), breastfeeding time (31 days versus 19 days, p < 0.001), enteral nutrition time (50 days versus 34 days, p < 0.001), weight gain (29 g/day versus 23 g/day, p = 0.002), and significantly lower respiratory support time (16 days versus 25 days, p < 0.001). Oxygen Exposure Time decreased but not significant (39 days versus 41 days p = 0.393). Parents hospital expenses in local Chinese RMB currency was not significant (84 K versus 88 K, p = 0.391). CONCLUSION: The results of our study suggests that FIC is feasible in two Chinese NICUs and might improve clinical outcomes of preterm infants with BPD. Further research is needed to include all infants admitted to NICUs and should include parent reported outcome measures. Our study may help other NICUs with limited parental access to implement FIC to enhance parental empowerment and involvement in the care of their infant.

RESUMEN

AIMS AND OBJECTIVES: To examine empirical studies of musical stimulation and music therapy carried out with preterm infants and their parents published from 2010-2015. BACKGROUND: Prematurity constitutes a global health problem that can impact the development of the preterm infant and the well-being of the parents. Music-based interventions may benefit the infant, parents and their relationship. In our review, we distinguished between musical stimulation and music therapy, as we found no previous studies that had made this distinction. DESIGN: This is a narrative literature review. METHODS: A search was undertaken in PubMed, PsycINFO and LILACS using the terms "music," "music therapy," "singing," "prematurity" and "preterm." Thirty studies were included and analysed according to the following categories: (i) aims of the study, (ii) participants, (iii) design, (iv) type of intervention, (v) assessment and measures and (vi) main results. RESULTS: The vast majority of the studies focused on the preterm infants and used an experimental design. Few studies carried out family-centred interventions, despite this having been noted as an important factor in effective interventions. Musical stimulation studies used more recorded music, whereas music therapy studies used more individualised interventions with live music. CONCLUSIONS: Both musical stimulation and music therapy demonstrated significant effects on preterm infants and their parents. However, compared to musical stimulation studies, interventions performed by music therapists provided more individualised care and tended to show greater effects on infants' physiological and behavioural responses. RELEVANCE TO CLINICAL PRACTICE: Our review showed that music therapy interventions may provide individualised, effective and family-centred care. There is a significant need for these types of interventions in the neonatal intensive care unit (NICU).
